This student regularly attends or is a member of your church and is applying to become a Mentor with Setting Captives Free. Please assist us by answering the following questions. The answers you provide below will be visible to both Setting Captives Free leadership and the student him/herself. If you prefer, you may alternatively fill out this form in PDF format (available here) and send it privately to us at scfapproval@gmail.com.

Question 1.​ Is the prospective Mentor an active member in good standing at your church?
